I authorize AKU Technologies, Inc. to obtain consumer reports concerning me from Checkr (www.checkr.com) for evaluating my eligibility to provide services through SHYNIT™ and, to the extent permitted by law, for periodic screening while I remain eligible to receive bookings. I acknowledge receipt of the separate Disclosure Regarding Background Investigation and any additional notices required by applicable law.
